Benign biliary strictures treated with percutaneous balloon dilatation and biodegradable stents - long term retrospective study

Popis Purpose: To find out which patients with benign biliary stricture treated with percutaneous balloon dilatation benefit from implantation of biodegradable stent. Methods: This single-centre retrospective study included 64 consecutive cases of PTD and balloon dilatation for benign biliary stenosis at a tertiary referral hospital from 1/2010 to 12/2022. Log rank test was used to assess recurrence of biliary strictures in patients with biodegradable stents (n=21) and without them. The variables included demographic (age, sex), laboratory (bilirubin, liver enzymes and inflammatory markers before the initial PTD and in 2-3, 6, 12, 24 and 36 months after extraction of all percutaneous instruments) and treatment parameters (treatment of recurrent stricture, extraction of biliary stones, number of dilatations, maximal balloon diameter, maximal balloon pressure, insertion of biodegradable stent, result of manometric test). Results: The recurrence of biliary stricture after treatment occurred in 20 cases (in 4 (25%) cases treated with biodegradable stent and in 16 (37%) cases treated only with ballon dilatation). Patients treated with balloon dilatation who have poor result of manometric test after balloon dilatation and patients with recurrent biliary stricture treated only with ballon dilatation have higher chance for recurrence of biliary stricture than patients treated with biodegradable stents (p=0.038). Other monitored variables were not significant. Conclusion: Patients with benign biliary stricture who have poor result of manometric test after balloon dilatation and patients with recurrent biliary stricture will benefit from implantation of a biodegradable stent.
