Detection of Treponema pallidum DNA from the Serum of an Adequately Treated Patient with Latent Syphilis.

Description The 29-year-old man developed signs of secondary syphilis, which involved a rash and lymphadenopathy. He was IgM-positive (Syphilis Captia IgM, Trinity Biotech, Ireland) and his initial RPR titre was 1:128. He was treated using penicillin G procaine, 1.5 million units twice daily for 14 days and afterwards using penicillin G benzathine intramuscularly, 2.5 million units weekly within three consecutive weeks. After penicillin treatment his RPR titre decreased rapidly. In June, the titre drop from 1:128 (in the end of March) to 1:4. At the same time, the patient was IgM-negative as was assessed by the ELISA method. Serological findings suggested clinical recovery. Despite the significant decrease of RPR titres we found in his serum T. pallidum DNA by polA PCR. This means we detected the DNA of T. pallidum from serum as long as 12 weeks after treatment.
